1.  CAREMARK NPI CHARGES

Effective 5/23/07 all claims submitted without an NPI number to Caremark, will be subject to a $.99/per Rx charge.  Caremark is not rejecting these claims and are just adding this charge to your submission.  Please check with your software company and make sure they have your store’s NPI number and that it is being submitted on your claims to avoid these charges.

Entering Physician NPI numbers is causing some claims to reject.  In these cases continue to submit the Physician DEA number until their NPI is required.

2.  REPORTING YOUR NPI TO NJ MEDICAID AND PAAD

If you have not already reported your NPI number to the state, you must fax your NPI number with your Provider Enrollment number to the state @ 609-584-1192.

3.  REPORTING YOUR NPI TO NY MEDICAID AND NY EPIC

Providers may enter the information on the http://www.eMedNY.org website. Providers or their representatives will need to have the NPI(s) they've obtained along with their eMedNY provider ID(s) and Tax Identification Number (TIN). The TIN will be a social security number or an employer identification number, as appropriate.
