National Provider Identifier (NPI)

To avoid payment delays, you will need to have this number on file with NCPDP

 by February 15, 2007.   This will ensure there is no interruption in your

 payment cycles from Third Party Payers.

A.  Have you applied for your NPI number (this number will replace the NCPDP number)? 

      Please list Pharmacy NPI Number _______________________ (IPA also needs this for Third Party Support)

        Please also list your current NABP #________________________ (For cross-reference only)

B.  If you have not yet applied (It takes about 15 - 20 min) for your PHARMACY NPI number, please do so immediately.

(PLEASE MAKE SURE you have your EMPLOYER ID #, MEDICARE & MEDICAID ID Numbers,

and your pharmacy STATE LICENSE information available)

1.     Go to the NPI enumerator website at:  https.//nppes.cms.hhs.gov/NPPES

   (Note: Do Not Use the forward or back buttons during this process or you will have to start over.)

If you have any questions during the process call 800-465-3203 and say/pick “NPI Specialist” and a rep will assist you.

2.      Select:  National Provider Identifier (NPI)

3.      Select:  Apply online for an NPI

4.      Select:  Begin Application Form

5.      Next:  Create an NPI User ID and Password (WRITE DOWN AND SAVE), then click next

6.      Follow the procedures indicated from that point on. 

7.      Provider Taxonomy (Provider Type/Specialty):

a.      Click “Add Taxonomy”

b.      Choose “33 Suppliers”

c.       Click “Next”

d.      Then choose the appropriate description of your pharmacy (retail, etc.)

e.      Enter your PHARMACY’s State License Number and state of issue

f.         Click “Save”

8.      Follow the system prompts for the rest of the application process.  At the end, the system will provide you with a Tracking Number (Save this in case you have future questions about your NPI.)

9.      Within 1 to 15 days, you should receive a Confirmation Letter via e-mail from the following e-mail address: customerservice@npienumerator.com.

10.  SAVE your confirmation letter in your NPI file.
